Case Management

The Case Management Services provided by our foundation are tailored to offer comprehensive support to students throughout their educational journey. By addressing a wide range of challenges they may face, we aim to ensure that each student receives the personalized assistance they need to thrive academically and personally.


Services: 


Individualized Needs Assessment and Goal Setting


  • Each student will undergo a thorough assessment to identify their unique needs, strengths, and challenges.
  • Based on this assessment, personalized goals will be collaboratively established to guide the student's educational path.


Development of Personalized Education Plans


  • Our team will work closely with each student to develop customized education plans that align with their academic goals and aspirations.
  • These plans will outline clear steps and milestones to help students stay on track and achieve success.


Referrals to Social Services, Counseling, and Healthcare Resources


  • Students will be connected with relevant social services, counseling, and healthcare resources to address any non-academic barriers they may encounter.
  • This may include access to mental health support, financial assistance programs, or community resources.


Advocacy and Support in Navigating Educational Systems and Accessing Financial Aid


  • Our case managers will advocate on behalf of students to ensure they have access to the resources and support they need within the educational system.
  • Assistance will be provided in navigating financial aid applications, scholarship opportunities, and other forms of financial assistance.


Regular Check-ins and Follow-up


  • Case managers will conduct regular check-ins with students to monitor their progress, address any emerging challenges, and provide ongoing support.
  • Follow-up sessions will be scheduled to review goals, adjust plans as needed, and celebrate achievements.
